Durable Power of Attorney for Healthcare Decisions
- A Durable Power of Attorney is a directive naming a person that you have chosen to make choices for you regarding life-sustaining measures and to follow your living will.
- “Health care decisions” are not limited to situations regarding the use of artificial life-sustaining procedures. “Health care decisions” include consent, refusal of consent or withdrawl of consent to any care, treatment, or service; or procedures to maintain, diagnose, or treat your physical condition.
- You may limit the powers your agent has.
- You may appoint an alternate agent to act if your first choice is unable to act on your behalf.
- Distribute copies to you agent, an alternates, and your primary physician
Living Will:
- A living will is a document where you will set forth your wishes in case you are unable to act for yourself to act for yourself to make health care decisions.
- The living will states your intension whether or not to withhold life-sustaining procedures.
- These include:
- Artificially supplied food or hydration.
- Withholding of treatment that only prolongs toe process of dying and is not necessary for your comfort or to alleviate pain.
